SERVICE PROPOSAL · EXAMPLE

What you receive after an inquiry

A service-proposal sample using fictional needs to show the structure of our work. It is not a client case, hospital acceptance or a quotation.

Example status: awaiting provider feedback

Your request

An example visitor seeks a specialist opinion in Chengdu, accompanied by one family member and needing English-language support. Dates and the receiving hospital are unconfirmed. No real patient information is used.

Institutions and open questions

We could first ask the relevant department at West China Hospital or Chengdu Western Hospital about intake, record requirements, in-person examinations, dates and language support. Listing a provider does not mean acceptance or a booked appointment.

Schedule around confirmations

Coordinator contacts the visitor → agrees required records and handover → receives provider feedback → confirms appointments and written costs → arranges transport and accommodation → accompanies visits → organises reports and handover home. Unconfirmed items stay pending; no assumed number of days is inserted.

Separate cost lines

Hospital: consultations, examinations, treatment and medicines under its plan. Panda Care: agreed coordination, interpreting and accompaniment. Third parties: accommodation, transport and optional activities. Each line identifies the payee, inclusions, exclusions, confirmation status and cancellation terms. This example contains no prices.

Who does what

Panda Care contacts providers, coordinates schedules, explains service scope and helps with handover. Hospitals and clinicians assess acceptance, determine care and provide clinical instructions. The client confirms goals, supplies requested accurate information, decides whether to accept the proposal and confirms payments and travel.

Before travel and before returning home

Before travel, confirm the campus, appointment notice, accompaniment contact, accommodation, transport and open items. Before returning home, organise reports, clinician instructions, follow-up and contacts; record how outstanding reports will be obtained.
